Power of Attorney and Guardianship Document Requirements

If you are the guardian of or have Power of Attorney (POA) for a member:

• If our office does not have the POA or guardianship papers on file for this member, please include a copy of your legal guardianship papers or POA papers when you return the form to the Retirement Systems so that we may process the change as soon as possible.
• Please note that a POA is no longer valid after the death of the person granting the POA.
• The Retirement Systems cannot provide legal advice. Please consult with an attorney if you have questions about a POA or guardianship.
