Document Checklist

​​When you're thinking about submitting a retirement application, whether online or by paper, you'll need to gather some documents beforehand to make it easier for you to complete your application. We've created a general Document Checklist to give you an idea of the kind of documents you'll want to have nearby as you complete your application forms. This is a general list, and some of these documents may not be applicable to your situation. Want a customized list of required documents? Complete an online application, and you'll be able to print a customized list based on the selections you make. Read below for more details.

You have two options for submitting your retirement application: an online retirement application or a paper application. You cannot start with one form of the application and complete it using the other application type. If you wish to change from an online application to a paper application, or vice versa, you will have to contact the Retirement Systems Division to cancel your application in order to start a new one, which may delay your retirement date. Be sure to work with your agency’s Human Resources staff before starting an application to make sure you’re following your employer’s retirement processes.

Online Retirement Application

If you’re using the online retirement application, you’ll want to download the Document Checklist and have copies of these documents handy when you complete Step 2 and 3. At the end of Step 2 of the online retirement application, you'll also be able to download and print a customized required documents checklist. This list is populated based on the answers you provide in Step 2. The Retirement Systems will need copies of each of the documents included on the customized list you will see at the end of Step 2 in order to process your application. If you do not have access to a printer when you initially see the link to the document checklist, you will be able to print the document at a later time.

Paper Application

If you're completing a paper application, you'll need to submit copies of these documents with your Form 6 application.
